Job Description:
A committed and compassionate Licensed Practical Nurse (LPN) is needed to provide high-quality nursing care within a skilled nursing environment. The LPN will support residents with daily clinical needs, ensuring safety, comfort, and adherence to care standards.
Responsibilities:
Provide routine nursing care in alignment with established care plans and facility guidelines.
Administer medications and treatments as prescribed, ensuring accurate documentation.
Monitor patient conditions, record vital signs, and promptly report changes to supervising RN or provider.
Assist with wound care, catheter care, and other clinical procedures based on competency and scope.
Support residents with ADLs when needed to promote comfort and well-being.
Maintain accurate and timely charting in compliance with facility and regulatory standards.
Collaborate with an interdisciplinary team—including RNs, therapists, CNAs, and providers—to ensure coordinated patient care.
Educate residents and families on care routines, medication administration, and safety practices.
Ensure a clean, safe, and supportive environment for all residents.
Uphold all state, federal, and facility regulations governing nursing practice.
Qualifications:
Active LPN license in the state of New Mexico (or ability to obtain prior to start).
Graduate of an accredited Practical Nursing program.
Experience in skilled nursing or long-term care strongly preferred.
Strong communication, clinical assessment, and documentation skills.
Ability to work collaboratively and deliver patient-centered care.
